BSW CHECKLIST AND DUE DATES

Newly accepted BSW students: This checklist provides links to important forms, documents, and information. Please familiarize yourself with the material contained in these links, and submit all forms on or before their due dates.

  Complete Major Declaration Form (PDF)

Fill out top half of form, sign it, and return it by April 7 to Judy Switzky or Belinda Velazquez, School of Social Work, 1350 University Ave., Madison WI 53706.

  Register for SW 440 and SW 441

Beginning April 8

  Review Field Education Handbook (PDF)

  Review Important Dates/Info for Field Students (PDF)

  Attend Field Forum

April 11, 1:00-3:30 p.m. at Union South

  Complete Wisconsin’s Background Information
      Disclosure Form (PDF)

Please include a check for $7.50 payable to the School of Social Work, and return form and payment by April 25 to Field Education Office, Room 318 Social Work Building, 1350 University Ave., Madison WI 53706.

  Complete Student Field Unit Preference Form (PDF)

Please return by April 25 to Field Education Office, Room 318 Social Work Building, 1350 University Ave., Madison WI 53706.

  Register for SW 400 (5 credits) on July 7

  Attend MSW/BSW Student Orientation on August 27
